T4A tax forms related to benefits Every year, the Operating Engineers’ Benefits Plan Office issues T4A Income Tax forms. The figure shown in Box 28— Other Income—includes the amount of premiums paid to the B.C. Medical Services Plan on your behalf and the amount of group life insurance premiums resulting from employer contributions during 2007. These are taxable benefits to you when paid by employer contributions from your hour bank. They should be included in your income when filing your income tax return. When the premiums for these benefits are paid through the Self-Payment provision of the plan, they are not included in the T4A amount. If you have any questions regarding your T4A when you receive it, please contact your Benefits Plan Office at 604-299-8341. Extended health claims and reimbursements You have until June 30, 2008 to claim reimbursement for eligible expenses incurred during 2007 on the Operating Engineers’ Extended Health Care Benefits Plan. Original receipts must be submitted with the completed claim form. Please call the Benefits Plan Office at 604-299-8341 for details. Notice to all crane operators All operators of tower cranes, boom trucks and mobile cranes in B.C. had to be registered by July 1, 2007. Contact the B.C. Association for Crane Safety for more information and fill out the online registration form or call 1-888-621-7233 or 604-276-3100.
